G-7: MID-SEMESTER EVALUATION

The Registrar will collect and record on the Integrated Student Information System (ISIS) faculty information on mid-semester course performance for first and second semester classification degree candidates and provisional students as of the sixth week of the Fall and Spring semesters. It will not include off campus or less-than 15 week courses.
Mid-semester evaluation scan forms containing the names of students in the identified course and section will be distributed by the Registrar prior to the sixth week of Fall and Spring semesters. Course instructors will complete the forms by identifying satisfactory (SAT) or unsatisfactory (UNSAT) for each student listed based on the student's achievement as of the sixth week of the semester. The completed forms will be returned to the academic unit or campus registrar by the established deadline.
Academic units, campus registrars will submit the evaluations to the Registrar by the announced semester deadline. Submission options include entering the evaluations to ISIS on-line or by returning the completed scan forms.
The Registrar will report unsatisfactory performance by mailing a notice directly to a student at his/her campus address (if not available, to the home address) by the end of the eighth week of that semester. Unsatisfactory evaluation information is also distributed to the student's college, Division of Undergraduate Studies or campus registrar. A second copy of the report is sorted by the student's academic adviser for further distribution to them.
Reports of unsatisfactory performance are also provided upon request to academic assistance centers for students associated with the centers.
Campuses with mid-semester evaluation systems that meet or exceed the requirements specified in this policy will be given the option of continuing their system or adopting the University-wide system.
The Registrar will certify each campus as having a mid-semester evaluation that meets or exceeds the requirements of the policy.
