Educational Planning Survey
The information you are asked to provide in this survey will be used during your academic advising day. The information will enable advisers working with the Division of Undergraduate Studies to discuss your academic plans with you. We are asking only for information that we have found useful in facilitating students' educational development and in establishing programs to foster effective academic advising. It is important to answer all questions.
Please complete Part I first, then go on to Part II immediately afterward. It will take you about 30–45 minutes to complete both parts.

After you have completed both parts of the Educational Planning Survey, please close this browser window and return to the FTCAP Web site for new first-year and provisional students (given to you in the FTCAP notification postcard and/or email that was sent to you) and take the placement tests online (if you have not already done so).
