Team # 664 -- UHS Pharmacy Team to Standardize Abbreviations, Acronyms, & Symbols on Handwritten Prescriptions
Student Affairs
August 2004
Objective
Objective: Use of traditional pharmaceutical abbreviations, symbols, and dose designations (sig codes) can result in prescription processing errors that compromise patient safety. JCAHO, as part of the National Patient Safety Goals initiative, has identified error-prone sig codes and recommended safer alternative sig codes. Goal: Increase the use of newly recommended sig codes for improved patient safety.Team Membership
