Objective

Coronary artery disease is a significant health problem and it has been reported that 1.2 million people in the United States experience an acute coronary event annually. The Penn State Heart & Vascular Institute and the Milton S. Hershey Medical Center Emergency Department are dedicated to improving the diagnosis and acute management of patients with acute coronary syndrome

Bringing together faculty and staff from the Milton S. Hershey Medical Center Emergency Department and the Penn State Heart & Vascular Institute has resulted in significant improvement in the treatment of patients presenting with acute ST-segment elevation myocardial infarction. The Heart Alert program has significantly impacted upon the treatment of patients with acute coronary syndrome by improving the “door-to-balloon” time, a clinical parameter that refers to timing of myocardial reperfusion starting at the moment the patient presents to the hospital.

Members of the Emergency Department (Dr. Craig Lauder, Dr. Chris DeFlitch, Dr. Greg Caputo) members of the Nursing Department along with PSHVI physicians and staff in the cardiac catheterization laboratory came together and focused on actions that would streamline the various processes that are essential in accurately diagnosing patients with an ST-segment elevation myocardial infarction. Once identified patients were enrolled in the Heart Alert program. The Admissions Department and staff from the Communications Department were also involved in triggering the heart alert system which when activated resulted in staff from the cardiac catheterization laboratory preparing for the patient’s arrival. The Penn State Heart & Vascular Institute interventional cardiologists (Dr. Charles Chambers-Director of the Cardiac Cath Lab, Dr. Ian Gilchrist and Dr. Mark Kozak) are valuable contributors in optimizing patient strategies for early reperfusion.

Changing protocols presented various challenges. However, all parties involved eagerly embraced these changes as the overriding intention of this program was to improve door to reperfusion time and patient care.

Oversight of the program continues in an effort to further optimize “door-to-balloon” times. Since the start of the program in September 2005, 99 patients have been enrolled with a mean door-to-balloon time of 91.0 min. Mean door-to-balloon time prior to the start of this program was 145 min. As of July 2007 36 patients have been enrolled with a mean door-to-balloon time of 82.9 min demonstrating that as the Heart Alert program becomes the standard of practice at HMC patient care continues to improve.

Results Achieved to Date
