Objective
A patient’s weight can have significant implications on multiple health issues. The purpose of this study is to evaluate how often patient weights are documented in the chart.Team Membership

The goal is to have a weight documented on 90% of active patient charts within a year of their most recent appointment. - Team Update
Weights are not being obtained by the intake staff at the target of 90% - only 67% of the time in October 2011. Weights obtained by the clinician cannot be added to the VS reportable data once the intake staff have signed it off. A new encounter would have to be created to document the wt/BMI in a reportable form. Will report the findings and reminder of the goal of a weight on each chart at least yearly at the February Clinical Services meeting.
