Team # 1012 -- Medical Problem List Updates for New Diagnoses

To determine whether 5 new diagnoses (PCOS, diabetes, eating disorders, depression and asthma) are added to the patient's problem list in the electronic health record. Accuracy of the problem list is important in providing continuity of care for the patient.

Results Achieved to Date

    The goal is for 100% accuracy in the problem list.
  • Team Update

    The data above indicates that clinicians are not always adding new diagnoses to the patient’s problem list. The diagnosis of depression, in particular, was only added 74% of the time. This may be, in part, to the reluctance of clinicians to formally document this particular diagnosis due to the possible repercussions from insurance companies. The number of clinicians documenting asthma as a diagnosis, on the other hand, was much better at 95%. The other three diagnoses fell in between these two extremes.

    The data was discussed with clinicians at a clinical staff meeting on February 15, 2012.
